All Orix Academy players travelling to Spain for the Donosti Cup are required to submit the following documents:

• Orix Waiver and Release of Liability Form
• Orix Travel Consent for Minors Form
• Two copies of the player’s travel insurance documentation

All documents must be completed and submitted on or before June 28, 2026.

The minimum travel insurance coverage required per player is $10,000,000 CAD

Document Submission Instructions:

• All documents must be submitted in paper form.
• Place all documents in a brown manila envelope.
• Clearly PRINT the player’s name on the outside of the envelope.
• Include two copies of your travel insurance documents.
• Do not seal the envelope.

All documents must be delivered in person.
Document submission deadline is Sunday June 28th
